For additional information regarding the request process, including documentation guidelines, please refer to the University Disability Support Services website. To initiate services, all new students must schedule and participate in an intake appointment, lasting about an hour. Students also need to complete the following steps in the DSS Portal: application of request, upload a completed impact statement, and upload documentation. The intake appointment provides an opportunity for discussion of accommodations and services offered by this office. Returning students must schedule a start-of-term meeting each term enrolled, typically lasting 30 minutes or less. Students should meet with a DSS professional no later than the end of the first week of the semester. At the start-of-term meeting, students have the opportunity to discuss accommodations and related issues. During the meeting, the DSS Staff will send notification to the faculty through the DSS Portal in the form of an electronic accommodation letter which will state the students approved accommodations. Immediately after the start-of-term meeting, students should email the professor to schedule a meeting. The meeting to discuss accommodations should be completed either in person or via zoom. Meeting with the professor is an excellent opportunity to discuss specific testing accommodations and to explore options if there are scheduling conflicts or other logistical problems. NOTE: The provision of accommodations is a shared responsibility between faculty members, the student, and the Disability Support Service. Therefore, it is vitally important that students meet with professors as early in the semester as possible. When meeting with faculty, students should discuss accommodations for the course because this will help to ensure accommodations are instituted in a timely manner. Faculty are not obligated to provide accommodations until receiving a students accommodation letter. Students requesting interpreters, alternative text formats, special seating, Braille, arrangements for personal assistants, use of equipment/technology, relocation of classrooms, and other accommodations requiring prior arrangement must request these services at least 6 weeks preceding the first day of class. Failure to request services in advance may result in delay of services. Services are not retroactive. Until students are officially registered, Disability Support Services cannot intercede with academic outcomes. Students requesting a review of approved accommodations should complete the Accommodation Review Questionnaire in the DSS Portal and upload a completed Accommodation Review Form. Students also need to contact the DSS office to request an appointment for further discussion of accommodation review. Additional documentation may be required to support revised accommodations. DSS Staff are available by appointment to discuss accommodations, academic difficulties, or other issues. We also offer one-to-one sessions in test taking, learning style and strategies, time management, career planning, assistive technology, and self-advocacy skills.
